Introduction: in addition to intraoperative processes, it is possible that the characteristics of the pituitary tumor are related to the prolonged postsurgical stay.


Objective: to determine the characteristics associated with prolonged post-surgical stay in patients operated on by transsphenoidal endoscopic surgery for pituitary adenoma.


Materials and methods: analytical and cross-sectional study of an international database of 704 adults. The variables were: post-surgical hospital stay, tumor type, tumor size, body mass index (BMI), physical status classification (ASA), age group. Cramer's V tests, Odds Ratio and prevalence ratio, sensitivity, specificity were used. , positive and negative predictive value, and decision tree by chi-square automatic interaction detector (CHAID)


Results: In men, the decision tree classified the null cell and lactotropic tumor type as a predictor of stay equal to or greater than 4 days. In women the associated characteristics were null cell, gonadotropic and lactotropic tumors. In men, the probability of prolonged postoperative stays was 4.01 times greater in those belonging to terminal node 2 (null cell tumor and lactotropes), with a frequency (PR) 2.58 times greater, while in women the probability and frequency 3.13 and 2.19 times higher.


Conclusions: the type of tumor is associated with prolonged post-surgical stays in a group of patients operated by transsphenoidal endoscopic surgery for pituitary adenoma. It is possibly due to harmful effects of the adenoma on underlying tissues, as well as systemic physiological mechanisms from the primary stages of the disease.
