Introduction. High blood pressure continues to be a challenge for public health in Paraguay, the lack of adherence to treatment is a problem from the medical, economic and psychosocial point of view. The use of technology is a useful tool in monitoring patients and improving adherence to pharmacological treatment.

Objetive. To evaluate adherence to antihypertensive pharmacological treatment in patients with hypertension who attend a public health service, through a digital telephone tool.

Materials and methods. Quasi-experimental study carried out in patients older than 18 years of both sexes with arterial hypertension; in which pharmaceutical interventions were applied through technology and adherence to treatment was evaluated using the Morisky-Green questionnaire, along with complementary questions to find out the causes. The research protocol was approved by a Research Ethics Committee of the FCQ-UNA (714/2021).

Results. 60 patients participated. 68.3% (n=41) at the beginning showed adherence to treatment according to the Morisky-Green questionnaire, and 31.7% (n=19) did not comply with the indication, after the interventions a greater adherence to treatment, reaching 96.7% of adherents (p<0.00001), being causes of non-adherence to pharmacotherapy forgetfulness 28.3% (n=17), lack of availability of the medication in the hospital 20 % (n=12) and cost 10% (n=6).

Conclusion. The use of technology could become a tool for primary prevention in the population with risk factors, and the monitoring of patients with arterial hypertension.
