Objetivo: identificar perfis clínicos e contextuais entre adolescentes trans no início do acompanhamento psicológico e examinar mudanças precoces no sofrimento psicológico durante as cinco primeiras sessões. Método: estudo observacional retrospectivo com 88 adolescentes trans (10-17 anos) atendidos em dois hospitais públicos chilenos. Foi realizada uma análise de classes latentes com seis indicadores: momento do processo identitário, afirmatividade familiar, bullying transfóbico, faixa etária, tentativas prévias de suicídio e diagnóstico clínico. O sofrimento psicológico foi avaliado pelo YP-CORE nas sessões 1, 3 e 5. O comportamento suicida foi mensurado pela Escala de Okasha. Resultados: emergiu-se uma solução de três classes com alta entropia (H = 0.961): (1) Revelação Identitária e Alta Proteção Relacional (12.5 %, n = 11); (2) Identidades Afirmadas em Contexto de Ambivalência Familiar (55.7 %, n = 49); e (3) Vulnerabilidade Clínico-Relacional Acumulativa (31.9 %, n = 28). Os perfis diferiram significativamente quanto ao sofrimento psicológico basal (H = 9.04, p = 0.011). Os perfis extremos apresentaram reduções significativas entre as sessões 1 e 3 (d = −0.66 a −1.01), enquanto o perfil com ambivalência familiar mostrou melhora mais gradual até a sessão 5 (d = − 0.50). Conclusão: os resultados evidenciam heterogeneidade clínica entre jovens trans e reforçam a necessidade de intervenções diferenciadas.
